29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition.  On or about October 23, 2013 and times prior thereto, employees of Church's Chicken located at or near 500 Oak Street in Conway, Arkansas were observed working in vicinity of a tripping hazard on the floor caused by  the floor tiles not being maintained in an orderly condition, specifically having several flooring tiles missing exposing employees to trip and fall hazards.

29 CFR 1910.22(a)(2): The floor of every workroom shall be maintained in a clean and, so far as possible, a dry condition. Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided where practicable.  On or about October 23, 2013 and times prior thereto, employees of Church's Chicken located at or near 500 Oak Street in Conway, Arkansas were observed working in the kitchen area of the restaurant that was adjacent to a triple basin sink that had drained water onto the floor surface exposing employees to slip and fall hazards.

29 CFR 1910.145(c)(3): Safety instruction signs. Safety instruction signs shall be used where there is a need for general instructions and suggestions relative to safety measures.  On or about October 23, 2013 and times prior thereto, employees of Church's Chicken located at or near 500 Oak Street in Conway, Arkansas were exposed to a kitchen floor that had become wet from water draining from a triple basin sink and there was no safety sign that had been placed in the vicinity to alert employees of this hazard.
