29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided: The employer is failing to protect employees using the proper personnel protective equipment from potential electrical hazards associated with hydro excavating near live power lines. The most recent example of this was found on the job site located at SW Corner of NW 1st St. & Highland Parkway Drive, Lincoln, NE 68510 where the employer had unprotected employees operating the Vermeer Model LP 855 SDT-J hydro excavator in effort to expose the live 8K volt underground power line without wearing the required electrical shock resistant gloves and boots thus exposing the employee to an electrical shock injury as evident in this accident. Instances include but are not limited to: i. Failure to provide electrical shock resistant gloves to prevent initial contact with unknown any live electrical parts. ii. Failure to provide electrical shock resistant boots to protect the employee from initial contact with unknown live electrical parts.
