29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) In the main shop, on or about 10/23/2013, guarding was not provided to protect employees from struck-by hazards created by flying metal debris and caught-in hazards created by rotating parts at the chuck when using the Jet Geared Head Precision Lathe (Model Number GH26120ZH, Serial Number 12420ZH0035). (b) In the main shop, on or about 10/23/2013, guarding was not provided to protect employees from struck-by hazards created by flying metal debris and caught-in hazards created by rotating parts at the chuck when using the Jet Geared Head Precision Lathe (Model Number GH2680ZX, Serial Number 080680ZX1903). (c) In the main shop, on or about 10/23/2013, guarding was not provided to protect employees from stuck-by hazards created by flying metal debris and caught-in hazards created by rotating parts when using the Daewoo Heavy Lathe (Model Number DST20, Serial Number 260030).
