29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  a.  The employer did not establish an energy control program consisting of equipment specific energy control procedures, employee training and periodic inspections exposing employees to an amputation hazard.

29 CFR 1910.219(c)(2)(i): All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires.  a.  In the saw mill, the edger drive mechanism, had a horizontal shaft that was not guarded exposing employees to a caught-in hazard.  b.  In the saw mill, the main saw drive mechanism, had a horizontal shaft that was not guarded exposing employees to a caught-in hazard.

29 CFR 1910.219(d)(1): Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.  a.  In the saw mill, on the edger machine, the exterior upper pulley was not guarded, exposing employees to a caught-in hazard.  The unguarded pulley was approximately 46 inches from the floor level.  b.  In the saw mill, on the edger machine, the exterior lower pulley was not guarded, exposing employees to a caught-in hazard.  The unguarded pulley was approximately 36 inches from the floor level.  c.  In the saw mill, the motor powering the edger machine, had a pulley that was not guarded, exposing employees to a caught-in hazard.  The unguarded pulley was approximately 24 inches from the floor level.  d.  In the saw mill, the edger machine ventilation unit, had a pulley that was not guarded, exposing employees to a caught-in hazard.  The unguarded pulley was approximately 24 inches from the floor level.  e.  In the saw mill, the motor powering the main saw, had a pulley that was inadequately guarded, exposing employees to a caught-in hazard.  The unguarded pulley was approximately 18 inches from the floor level.  f.  In the saw mill, the pulley that rotated the main saw was not guarded, exposing employees to a caught-in hazard.  The unguarded pulley was approximately 36 inches from the floor level.

29 CFR 1910.219(e)(1)(i): Where both runs of horizontal belts are seven (7) feet or less from the floor level, the guard shall extend to at least fifteen (15) inches above the belt or to a standard height, except that where both runs of a horizontal belt are 42 inches or less from the floor, the belt shall be fully enclosed in accordance with paragraphs (m) and (o) of this section.  a.  In the saw mill, the edger machine had a double run of horizontal belts connecting the lower and upper pulleys hat was not guarded exposing employees to a caught-in hazard.  b.  In the saw mill, the edger machine had a double run of horizontal belts connecting the motor and upper pulley that was not guarded exposing employees to a caught-in hazard.  c.  In the saw mill, the edger machine had a double run of horizontal belts connecting the motor and ventilation unit that was not guarded exposing employees to a caught-in hazard.  d.  In the saw mill, the main saw had run of eight horizontal belts connecting the motor and drive pulley that was not guarded exposing employees to a caught-in hazard.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets.  a.  In the saw mill, on the side of the green line, a chain and two sprockets were not guarded, therefore exposing employees to a caught-in hazard.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  a.  In the saw mill, near the main saw, the 220v electrical outlet used for the welder did not have a faceplate, exposing employees to electrical contact.
