29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Inside the machine shop, on or about October 24, 2013, guarding was not provided to protect employees from struck-by hazards created by flying metal debris and caught-in hazards created by rotating parts at the 16" chuck when using the Vantage Machine Lathe (Unit Number 18, Model Number3A-3080).

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: a) Inside the machine shop, on or about October 24, 2013, the main power outlet on the Victor Lathe (Model Number 1640, Serial Number 56442) lacked strain relief, exposing uninsulated covered conductors.
