29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) At 3710 South Sam Houston Parkway East in Houston, Texas, on or about 10/25/2013, employees engaged in clearing gutters on a PODS Storage Facility, more than 6 feet above the ground, were not provided with any type of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) At 3710 South Sam Houston Parkway East, located in Houston, Texas, on or about 10/25/2013, employees accessing and egressing a roof while performing work used a Werner 32' aluminum extention ladder whose siderails did not extend past the landings surface at least three feet.
