OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards:         a)  2758 Gunter Park Dr. West, Montgomery AL - Production Floor; On or about October 22, 2013 and at times prior thereto the employer exposed employees to struck-by hazards in that employees were allowed to walk and work around steel storage racks that had damaged / bent members and column base plates that were not anchored to the floor.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   a)  Production Floor; On or about October 22, 2013 and at times prior thereto the employer exposed his employees to potential hazardous energies in that employees were allowed to work on the Extrusion Presses which have multiple energy sources such as electric, air, natural gas, water, and nitrogen without those employees having a written machine specific lock out procedure to ensure that those energy sources were controlled while the employees were performing maintenance or servicing on the machine.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Production floor; On or about October 22, 2013 the employer exposed an employee to an amputation hazard in that the employee was allowed to operate the #26 Crown Machine with the guard open over the incoming straightening rollers.  b) Production Floor: On or about October 22, 2013 the employer exposed an employee to an amputation hazard in that the employee was allowed to operate the #27 Crown Machine with the guard open over the incoming straightening rollers.  c) Production floor; On or about October 22, 2013 and at times prior thereto the employer exposed employees to amputations hazards in that employees were allowed to work and walk around an unguarded breakdown spool in the coating room.
