29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a) Roof - On or about 10/23/2013, two roofers were operating a pneumatic nail gun without eye protection. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: Roof - On or about 10/23/2013, two roofers were exposed to a 22 foot fall from the unguarded platform of a pump jack scaffold system which had not been provided with standard guardrails or any similar and acceptable form of fall protection.
