29 CFR 1910.119(e)(1): The employer shall perform an initial process hazard analysis (hazard evaluation) on processes covered by this standard. The process hazard analysis shall be appropriate to the complexity of the process and shall identify, evaluate, and control the hazards involved in the process.   At the time of the inspection, the employer had not a hazard evaluation for the ammonia refrigeration system that contained apprximately 14,414 pounds of ammonia.  Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):  At the time of the inspection, the belts and pulleys of the #4 reciprocating compressor were not guarded as required by this section.  Abatement certification and abatement documentation is required for this violation. 29 CFR 1910.307(c): Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location:  At the time of the inspection, the electrical wiring in the engine room was not approved for Division 1 class 2 hazards resulting from the potential exposure to explosive levels of ammonia.   Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.
