29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.  a) On or about 10/28/13, in the machine shop areas, Wood and Aluminum dusts accumulations on machines' surfaces such as, but not limited to: radial saw, table saw, electrical junction box and on floors were greater than 1/32 inch and required cleaning.  b) On or about 10/28/13, in the machine shop areas, a 600 volt cord from a Dri-Air Machine was on an aisle floor creating a tripping hazard.   ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) On or about 10/28/13, in the establishment, employees performed servicing/maintenance of machines, such as, but not limited to: changing dies on a Multi-Press Shear Press, Model No WZAP-3, Serial #30907 and a Nissei Injection Molding Machine, without the use of an energy control program (Lockout/Tagout(LOTO) Program), machine-specific LOTO procedures, hardware, training or periodic inspections.   ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use:  a) On or about 10/28/13, in the battery charging area, there was not an emergency eye wash immediately available for employees refilling water on forklift trucks' batteries. These batteries contained corrosive acid.  ABATEMENT CERTIFICATION REQUIRED
