The log is missing the limbs identification, i.e. left or right finger. The following entries are missing the required information for 2013. 1. "Cut-Puncture limb-Finger by drill" 2. "Limb-Sprain Limb-Elbow by" 3. "Employee while grinding down a nut when he powered down tool metal cut-off wheel he lacerated finger"

29 CFR 1910.134(k)(1)(iv): The employer did not ensure that each user could demonstrate knowledge of how to inspect, put on, remove, use, and check the seals of the respirator: On or about October 28, 2013, at the above mention job site, employees that work in the anodizing department with corrosive chemical like sulfuric acid and sodium hydroxide did not demonstrate the proper procedure of how to perform the seal check on their half face respirator.

29 CFR 1910.138(b): The employer did not base selection of appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the tasks to be performed, conditions present, duration of use, and the hazards and potential hazards identified: On or about October 28, 2013, at the above referenced job site, an employee was not provided the proper protection to use cleaning chemical such as but not limited to alcohol and acetone. The employee was not wearing of chemical resistant gloves.
