OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load. a) Concrete, ice block machine - 8/13/2013- Below-the hook lifting device used to lift ice blocks and ice block forms had no rated load capacity markings or manufacturer's specifications. b) Ice carving block area- On and before 10/18/13 - Below-the hook lifting devices attached to Clinebel hoists had no rated load capacity markings or manufacture's specifications. c) Concrete, Ice Block Machine - On or about 08/13/13 - An overhead carrier beam rail with an attached 2 ton Coffing JLC hoist used to lift ice blocks was not rated, tested or inspected.
