29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: Location: 401 Holly Hills Avenue, St. Louis, MO 63111, P-Lot An electrician was working from a JLG Articulating Boom Lift, 450AJ Series II, identification number J12069, without a personal fall arrest system (PFAS). The employee was approximately twenty feet above the asphalt parking lot performing maintenance to the parking lot pole lighting fixtures.
