Location: 325 West Broadway, NY NY - Building East side - basement, 1st, 2nd and 3rd floors On or About October 25, 2013 a) Temporary lighting attached to handrail of stairwell and contained socket with broken bulb and socket with missing bulb. Stairs located in back building, east side of site. b) Temporary lights for general illumination were not protected from accidental contact by employees by a cage or cover throughout the site. NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.

Location: 325 West Broadway, NY NY -2nd floor interior side next to job-made ladder: On or About October 25, 2013 a) Employees exposed to a fall of approximately 12 feet while stripping columns on the 2nd floor. An employee was injured when he fell from the 2nd floor to the ground floor through an area missing a guardrail. NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.
