29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: A) Silgan Plastics, located at 690 Woodland Drive, Ottawa, OH 45875: On or about October 28, 2013, the employer failed to utilize machine-specific energy control procedures to control hazardous energy of the punch press on Line 11 (equipment # 21005). Employees did not control all hazardous energy sources such as, but not limited to, electrical and pneumatic, while performing activities such as, but not limited to, clearing jams and cleaning the press. Employees were exposed to amputation hazards. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: A) Silgan Plastics, located at 690 Woodland Drive, Ottawa, OH 45875: On or about October 28, 2013, the employer did not ensure that the punch press on Line 11 (equipment # 21005) was properly turned off/shut down while employees conducted activities such as, but not limited to, clearing jams and cleaning the press. Employees were exposed to injury from unexpected start-up and energy release. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

Employees were exposed to amputation hazards. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
