29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two: a) At the jobsite on or about 10/23/13: Employees described the horizantal lifeline system installed to use when sheathing the roof. This system was an installed cable attached at three points (each gable end and in the center of the roof ridge), where multiple employees would attach at the same time. The employee who installed the system was not able to describe the strength of the anchorages or the safety factor. Abatement certification must be submitted for this item.
