29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: The employer failed to protect employees from struck by hazards associated with operating a powered industrial truck with modifications that change the performance of the vehicle. This was most recently evident during the inspection of the facility located at 6363 N. 70th Street, Lincoln, NE 68509, where the employer has taken the forks that came with the forklift and inverted them so as to increase the lifts height lifting capability without having researched with the manufacturer to determine that the change will not have an adverse effect to the lifts capability.
