29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:       On or about October 29, 2013, employees performing framing work were not protected by a protective helmet while working underneath employees that were framing above at higher levels. The employees were exposed to being struck by objects and/or tools falling from a height greater than 25 feet.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stair rail system along each unprotected side or edge:     On or about August 29, 2013, employees using a stairway that was not equipped with at least one handrail along each unprotected side to access the upper levels of a residential home under construction were exposed to an unprotected fall of up to 8 feet to the concrete below.
