29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  a.    On or about October 29, 2013, employees working on the east side of the shop were using a Black & Decker dual wheel bench grinder with the work rests on the both sides of wheels adjusted with an opening or gap greater than an 1/8 inch gap, exposing employees to the hazards of flying debris.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  a.   On or about October 29, 2013, on the east side of the Shop, the tongue guards on the right and left grinding wheels on the Black and Decker bench grinder were adjusted so that distance was greater than 1/4th inch from the surface of the grinding wheels and the adjustable tongue guards, exposing employees to the hazard of flying debris.

On or about October 29, 2013, the employer had not developed and implemented a written hazard communication program to outline how requirements of 29 CFR 1910.1200(f), (g), and (h) would be met for employees who had exposure to hazardous materials such as but not limited to oxygen-fuel gas, acetylene, Motor oil and Radnor Tubular welding wire.
