29 CFR 1910.134(g)(1)(i)(A):  Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function:       Employees working with volatile solvents were required to wear tight-fitting, half-face, air purifying respirators and the employer did not ensure those employees required to wear a respirator maintained a seal surface that was not interfered by facial hair.

29 CFR 1910.178(c)(2)(iv):  Power-operated industrial trucks designated as DY, EE, or EX were not used in locations where volatile flammable liquids or flammable gases were  handled, processed or used, but in which the hazardous liquids, vapors or gases were  normally confined within closed containers or closed systems, from which they could have escaped only in case of accidental rupture or breakdown of such containers or systems, or in the case of abnormal operation of equipment:       Employees performing fiberglass mixing and equipment cleaning in the Mix Room and EMC Room were exposed to fire hazards and the employer did not ensure power-operated industrial trucks (forklifts) designated as DY, EE, or EX were used in locations where volatile flammable liquids were handled but in which liquids or vapors were normally confined within closed containers or systems and could have escaped only in case of accidental rupture or breakdown of such containers or systems or in the case of abnormal operations of equipment.

29 CFR 1910.141(a)(3)(ii):  Floor(s) of workroom(s) were not maintained, so far as practical, in a dry condition:       Employees performing press and shipping operations at the following areas were exposed to slip hazards during rainwater entering the building through roof leaks and the employer did not maintain floors so far as practical in a dry condition:       A) Employees performing press operation in the area of Press 1, 2, and 3 were exposed to slip hazards, and      B) Employees performing shipping operations in the area of the shipping department computer station were exposed to slip hazards.      The Swan Corporation was previously cited for a violation of this Occupational Safety and Health Standard, or its equivalent standard, 29 CFR 1910.141(a)(3)(ii), which was contained in OSHA Inspection Number 331312,  Citation 1, Item 1, and was affirmed as a Final Order on June 12, 2012, with respect to a workplace located at 200 Swan Avenue, Centralia, IL 62801.

29 CFR 1910.22(d)(1):  On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed to the building:    Employees performing fiberglass mixing were exposed to overloading hazards including but not limited to caught-between and struck-by hazards and the employer did not ensure approved loads were marked on plates affixed to the mezzanine area in the mix room.
