29 CFR 1926.652(a)(1):  Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c):       (a)   Cap Excavating, Inc. at 15587 Huron St., Broomfield, CO 80023:  On and before October 25, 2013, the employer did not ensure that each employee conducting work in an excavation was protected from cave-in, in that work performed to uncover a water line main pipe was done in the area of the trench end-wall that was approximately 8 feet deep and lacked a protective system.  This condition exposed employees to a crushing and struck-by hazard by excavation wall cave-in.

29 CFR 1926.200(g)(2):  The employer did not ensure that all traffic control signs or devices used for protection of construction workers conformed to Part VI of the Manual of Uniform Traffic Control Devices (MUTCD) Millennium Edition, December 2000:    (a)   Cap Excavating, Inc. at 15587 Huron St., Broomfield, CO 80023:  On and before October 25, 2013, the employer did not ensure that each employee conducting work in the shoulder of a two lane street were protected by advanced warning signs.  This condition exposed employees to struck-by hazards from motor vehicles.  The speed limit on the street was 45 mph.
