29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:       A. C & J Custom Construction LLC worksite located at 3675 Turtle Creek Dr., Perrysburg, Ohio: On or about October 29, 2013, the employer did not ensure employees wore proper eye protection while operating a pneumatic nail gun and circular saw to cut and secure cement board  The employees were exposed to potential flying object hazards while operating the pneumatic fastener and saw.

29 CFR 1926.453(b)(2)(v): A body belt was not worn with a lanyard attached to the boom or basket while working from an aerial lift:      A. C & J Custom Construction LLC worksite located at 3675 Turtle Creek Dr., Perrysburg, Ohio: On or about October 29, 2013, the employer did not ensure employees wore personal fall arrest systems while operating the Genie S-60 X Aerial Lift. The exposed employees were installing siding on the south side of the residential structure.
