C & J Custom Construction LLC worksite located at 3675 Turtle Creek Dr., Perrysburg, Ohio: On or about October 29, 2013, the employer did not ensure employees wore proper eye protection while operating a pneumatic nail gun and circular saw to cut and secure cement board The employees were exposed to potential flying object hazards while operating the pneumatic fastener and saw.

C & J Custom Construction LLC worksite located at 3675 Turtle Creek Dr., Perrysburg, Ohio: On or about October 29, 2013, the employer did not ensure employees wore personal fall arrest systems while operating the Genie S-60 X Aerial Lift. The exposed employees were installing siding on the south side of the residential structure.
