(Maritime Reference 29 CFR 1915.91)    a) On barge DBL-119 moored at the facility, cargo tank number 1: On October 27, 2013, danger tags were not used to prevent accidental injury during repair work in and around cargo tank number 1, cargo piping and vent lines filled with nitrogen, exposing employees to an asphyxiation hazard.       b) On barge DBL-119 moored at the facility, cargo tank number 2: On October 27, 2013, danger tags were not used to prevent accidental injury during repair work in and around cargo tank number 2, cargo piping and vent lines filled with nitrogen, exposing employees to an asphyxiation hazard.      c) On barge DBL-119 moored at the facility, cargo tank number 3: On October 27, 2013, danger tags were not used to prevent accidental injury during repair work in and around cargo tank number 3, cargo piping and vent lines filled with nitrogen, exposing employees to an asphyxiation hazard.

29 CFR 1915.12(a)(1)(i): The employer did not ensure that sealed and non-ventilated spaces were visually inspected and tested by a competent person to determine the atmosphere's oxygen content prior to initial entry into the space by an employee.      a) On barge DBL-101 moored at the facility, cargo tank number 1: On October 27, 2013, employees were directed to enter a closed, sealed confined space without visual inspection and testing by a competent person to determine the atmosphere's oxygen content, exposing employees to an asphyxiation hazard.       b) On barge DBL-102 moored at the facility, cargo tank number 2: On October 27, 2013, employees were directed to enter a closed, sealed confined space without visual inspection and testing by a competent person to determine the atmosphere's oxygen content, exposing employees to an asphyxiation hazard.     c) On barge DBL-119 moored at the facility, cargo tank number 3: On October 27, 2013, employees were directed to enter a closed, sealed confined space without visual inspection and testing by a competent person to determine the atmosphere's oxygen content, exposing employees to an asphyxiation hazard.
