29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a) M.J. Brown Co., Inc. on or about October 29, 2013, at the site of building 18050 on the Camp Perry Complex in Port Clinton, OH: The employer failed to ensure employees working from an aerial lift were adequately protected from falls. Employees working from the lifts were not clipped off to the basket while the platform was elevated. The employees were exposed to falls up to 18 feet to the ground below.
