29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) Netherwood Pointe - 920 South Avenue, Plainfield, NJ 07061: The ladder provided was not secured and did not extend 3 feet above the working surface of the 2nd floor to allow for safe access on and off the 2nd level. Condition observed on or about October 30, 2013.
