29 CFR 1910.133(a)(1): Protective face equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:   (a)  At the foundry:  The employer did not provide protective face equipment to protect employees from skin burn hazards, while melting and processing iron. Employees were not wearing face shield to prevent 2nd and 3rd dregee burns from the melting process.

29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces:    (a)  At Clay and Bailey Manufacturing Company:  The employer failed to evaluate and determine which confined spaces are permit-required and which are not permit-required in the workplace.         1) On November 13, 2013,: Employees were potentially exposed to carbon monoxide, oxygen deficiency, and fall hazards while performing maintenance and cleaning activities inside the elevator pit.
