29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a- 1600 Rookery Court, West Windsor NJ: On or about 10/29/13 company employees were exposed to falls greater than 20 feet 8 inches to the ground surface below when the scaffold's walking/working surface, that they were working from, contained 2 - 3 planks on a system that requires 5 - 6 planks.

29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph.  a- 1600 Rookery Court, West Windsor NJ: On or about 10/29/13, company employees were exposed to falls greater than 20 feet 8 inches to the ground surface below when working from a prefabricated, tubular, welded framed scaffold that was not restrained from tipping.  Employees attempted to counterbalance scaffold using cinderblocks placed on ends of the scaffold's lower frames.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(e)(6)(vi): 1926.451(e)(6)(vi):  Integral prefabricated scaffold access frames being used for access had a spacing greater than 16 3/4 inches.  a- 1600 Rookery Court, West Windsor NJ: On or about 10/29/13, company employees were exposed to falls greater than 20 feet 8 inches to the ground surface below when they were climibing the rungs of the scaffold's frames to access and egress the walking/working surface.  The spacing between the individual rungs was approximately 18 inches.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a- 1600 Rookery Court, West Windsor NJ: On or about 10/29/13 company employees were exposed to falls greater than 20 feet 8 inches to the ground surface below when they were not provided with any fall protection, guard rails, safety nets, endstops, etc.  At the time of the exposure these employees were performing red exterior brick installation and other related activities from a prefabricated, tubular, welded framed scaffold's third and forth buck (as well as from the fourth buck's side bracket).
