29 CFR 1910.178(l)(3): Powered industrial truck operators did not receive initial training in the following topics, except in topics which the employer can demonstrate are not applicable to safe operation of the truck in the employer's workplace, (A): Operating instructions, warnings, and precautions for the types of truck the operator will be authorized to operate: Facility: The employer did not ensure that stable and safely arranged loads were properly handled by a powered industrial truck. The Operator used the forks of machine to remove defective product from a roll attached to a Slitting machine. The forks moved or slipped and caused an injury to a worker's hand that was very close to the point of operation.
