29 CFR 1926.601(b)(4): The employer had in use motor vehicle equipment having an obstructed view to the rear which did not have a reverse signal alarm audible above the surrounding noise level or an observer signaling it was safe to back up the vehicle: (a) On or about 09/20/2013, while installing highway guardrails at the construction of Interstate 4 in Tampa, the Dodge Ram 3.5 Ton Crew Truck did not have an audible backup alarm, or an observer, to warn pedestrians of impending equipment traveling in reverse.

A) As observed on or 09/20/2013, an accident prevention program suitable to protect the employees from the risks of construction work was not implemented. No formal job site inspections, disciplinary policies or safety orientations were observed.
