29 CFR 1926.451(b)(5)(i): The end of a platform 10 feet or less in length, extended over its support more than 12 inches:Note: Does not apply if the platform was designed and installed so that the cantilevered portion of the platform was able to support employees and/or materials without tipping or had guardrails that blocked employee access to the end. On or about October 28, 2013, on the east side of the construction site, employees engaged in masonry activities on a residential home worked from a tubular welded frame scaffold where the planks extended over 12 inches past the bearer exposing employees to a fall hazard of 13 feet to the ground below.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. On or about October 28, 2013, on the east side of the construction site, employees engaged in masonry activities on a residential home worked from a tubular welded frame scaffold without fall protection and were exposed to a fall hazard of approximately 13 feet to the ground below.
