29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: On or about October 28, 2013, in front of the house on a multi-pitched roof, two employees were observed working without utilizing any fall protection system. Employees were exposed to a fall hazard of greater than six feet in the following instances: a) One employee was working from the entryway roof plastering the front side of the house. b) One employee was working from the garage roof preparing chicken wire for plaster. c) Both employees used a single scaffold plank to crossover from roof to roof; the crossover was approximately 40-inches wide.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about October 28, 2013, the 8-foot, Werner extension ladder positioned front and to the left of the garage, did not have the ladder side rails extend at least 3 feet (.9 m) above the roof surface. Employees were exposed to fall hazard of approximately 10-feet 9-inches.
