29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     (a)  On or about 10/29/2013, at the west side of the residence, employees were installing a new roof and were not provided fall proteciton.  The employees were exposed to falls of more than 6 feet to the ground below

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder length allowed, the ladder side rails did not extend at least 3 feet (0.9 m) above the upper landing surface being accessed:     (a) On or about 10/29/2013, on the back of the house, employees used a Louisville aluminum extension ladder with side rails that did not extend at least 3 feet above the edge of the roof to access the roof.  The employees were exposed to falls of approximately 8 feet to the ground below.

29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall:    (a) At the front of the house, an employee was carrying a bundle of roofing shingles while climbing the ladder, exposing the employee to fall hazards of approximately 8 feet to the ground below.
