29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) For the construction work taking place at 2010 E. Hillsborough Ave., Tampa, FL, as observed on or about 10/31/2013. Employees working on top of a sign that was over 20-feet high were tied of with a fall arrest system that had 10-15 feet of lifeline at the employees' working position. This resulted in over 15-feet of free-fall before the deceleration device would engage. The deceleration device used on the employees' lanyards, which were part of a roofer's fall protection kit, was rated for a free-fall of only six feet. Employees were exposed to potential trauma and severe internal injuries from the arresting forces of a free-fall of over 15-feet. This configuration also exposed employees to swing-fall hazards of striking the building/sign or ladder during the fall.
