29 CFR 1910.147(c)(7)(i)(C): Employees whose work operations were in an area where energy control procedures were utilized were not instructed about the procedure:    (a) There was no lockout training for affected and other employees such as, but not limited to, coater operators and temporary employees with certification records maintained, about the procedures and about the prohibition relating to attempts to restart or re-energize machines or equipment which are locked out.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) In the Coater Room on #8 roller coater, the ingoing nip point of the paint roller was inadequately guarded in that the top guard was hinged and not secured with fasteners that were not readily removable or interlocked to stop the drive mechanism. In addition, the ingoing nip point on the board feed side was inadequately guarded in that the feed opening in the guard was large enough that the operator and other employees could reach the nip point and the guard opening did not meet Table O-10.    (b) In the Coater Room, the 2 other roller coaters also had ingoing nip points that were inadequately guarded in that the top guards were hinged and not secured or interlocked, plus the board feed guard openings were too large and did not meet the maximum allowed by Table O-10.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:    (a) In the Coater Room, the chain and sprocket drive for #8 roller coater was inadequately guarded in that the guard was not secured in place with fasteners that were not readily and easily removed in that the guard was just hung on the side guard frame.  (b) In the Coater Room, the chain and sprocket drives for all the other roller coaters were also inadequately guarded in that they were also not secured and were just hung on the side guard frames.
