29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    On October 30, 2013, at the above addressed site, an employee was exposed to a struck by hazard when using a circular saw and pneumatic nail gun while not wearing eye protection equipment.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On October 30, 2013, at the above addressed site, an employee was exposed to a fall hazard performing carpentry work on a walking/working surface approximately 10 feet above a lower level while not being protected from falling to that lower level.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On October 30, 2013, at the above addressed site, employees performing carpentry work exposed to potential falls were not provided with a training program that would enable them to recognize the hazards of falling.   PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On October 30, 2013, at the above addressed site, an employee performing carpentry work was exposed to a fall hazard while using an aluminum extension ladder that was not extended at least 3 feet above the upper landing surface to which he was gaining access to.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:  On October 30, 2013, at the above addressed site, employees performing carpentry work were exposed to a fall hazard while using an aluminum extension ladder set up in an active construction site that was not secured to prevent accidental displacement.
