29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) West Building, Buffing OD work area, a four-plex junction box that was powering extension cords for equipment including a laptop, a charger for a Makita drill, and the scan station where employees were using computer terminals to log in parts was hanging by the power cord in lieu of mounting the box, exposing employees to the hazards of electrical shock. (b) West Building, at a table in the Buffing OD work area, a Cyber power surge strip used to power a laptop and battery charger for a Makita drill was series connected (daisy chained) to an extension cord that was plugged into a four-plex junction box that was suspended by its cord, exposing employees to the hazards of electrical shock.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: (a) West Building, Buffing OD work area, flexible cord powering the computer at the scan station where employees were using terminals to log in parts was plugged into an extension cord that was zip-tied to a steel eye mounted on top of a cabinet, draped over the steel support for overhead lights, and plugged into a junction box that was suspended by its cord, exposing employees to the hazards of electrical shock. (b) West Building, at a table in the Buffing OD work area, an extension cord powering a Cyber power surge strip was plugged into a suspended 4-plex outlet receptacle, draped over the steel support for overhead lights, and dropped along the wall behind the scan station where employees were logging in parts before polishing, exposing employees to the hazards of electrical shock.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (a) In the West Building, buffing cells area, on or about August 5, 2013 and at times prior thereto and thereafter, information and training on hazardous chemicals was inadequate in that employees were not aware of dermal hazards of polishing compounds and aluminum dust, and were not informed of the measures they can take to protect themselves, such as personal protective clothing, exposing employees to the hazards of dermal contact with hazardous chemicals. (b) In the West Building, buffing cells area, on or about August 5, 2013 and at times thereafter, information and training on hazardous chemicals was inadequate in that employees were not aware of health hazards of dust generated during buffing operations on aluminum parts with aluminum oxide polishing compounds, and were not informed of the measures they can take to protect themselves, such as use of respiratory protection, exposing employees to the hazards of chemical inhalation.
