29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: An employee working on a tubular welded frame scaffold was exposed to falling approximately 15 feet to the ground below due to the lack of guardrail systems installed on all open sides.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. An employee working on a tubular welded frame scaffold was exposed to falling approximately 15 feet to the ground below and the employer had not provided training to the employees on the hazards associated with working off of the scaffolding and procedures to minimize those hazards.
