29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: Radiology Storage, Back Hallway: On or about October 30, 2013 and at times prior thereto, one (1) "Tripp Lite" relocatable power tap (RPT) was connected in series to a grounded extension cord to provide power to the Pyxis machine. This condition of use was contrary to the associated UL listing and labeling when using the equipment and exposed employees to smoke-inhalation hazards in the event of RPT failure.

29 CFR 1910.305(g)(1)(iv)(C): Flexible cords and cables were run through doorways, windows, or similar openings: Radiology Storage, Double-doors: On or about October 30, 2013 and at times prior thereto, the flexible cord for Tripp Lite relocatable power tap (RPT) was routed under the door to provide a power source for the Pyxis machine in the back hallway. This condition was not in conformance with the use of flexible cords and exposed employees to electrical shock hazards from potential damage to the outer jacket of the 120 volt power cord.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: Radiology Storage Room: On or about October 30, 2013 and at time prior thereto, a red, contractor-supplied extension cord was in use with damaged strain-relief on the attachment plug. This condition exposed employees to electrical shock and burn hazards when disconnecting the extension cord from the 20 amp, 120 volt service.

29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association (CGA) Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: Basement, Gas Cylinder Storage: On or about October 30, 2013 and at times prior thereto, the nitrogen and nitrous oxide gas cylinder storage and manifold room was not prominently posted with the gases stored. This condition was not in accordance with CGA Pamphlet P-1-1965 paragraph 3.3.2.
