29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE).  Maintenance Garage:  On or about 29 October 2013 and times prior thereto, employees were operating gas powered chainsaws without a hazard assessment identifying what PPE was required for the task.  This condition exposed employees to caught in hazard by operating chain saws without chaps.

29 CFR 1910.176(a): Aisle(s) and passageway(s) were not kept clear and in good repair with no obstruction across or in aisles that could create a hazard.    Tank Storage Room Area:  On or about 29 October 2013 and times prior thereto, the aisle way adjacent to upper deck area measured approximately 16 inches.  This condition exposed employees to delays in existing the building creating tripping, smoke inhalation and fire hazards.

29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors were not replaced.      Welding Shop area:  On or about 29 October 2013 and times and prior thereto, the Hobart welder, Model TR-300, serial number 2RT-17010, had cut insulation approximately 6 feet from the stinger.  This condition exposed employees to an electrical shock and burn hazard.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident.    Switch Gear Room:  on or about 29 October 2013 and times prior thereto the electrical panel beside panel BX2A was not labeled to identify what circuit was controlled by the panel.  This condition exposed employees to a 120 volt electrical shock and burn hazard.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service    Boiler Room:  On or about 29 October 2013 and times prior thereto, a 120 volt Milwaukee 1/2" Impact Wrench, model number 9066, serial number 754A197100140 had a damaged cord exposing wiring.  This condition exposed employees to an electrical shock and burn hazard.       The Alaska Native Tribal Health Consortium was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 307504449, citation number 1, item number 3 and was affirmed as a final order on April 21, 2009, with respect to a workplace located at Alaska Native Tribal Health Consortium.
