Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards from an ignition source located in the area of flammable liquids and vapors while removing a dump valve assembly from a separator containing flammable condensate and vapors: (a) Jonathon Davis Pad: Employees were removing a dump control unit from the low side of a three phase horizontal separator when flammable condensate was released from the vessel and ignited. A burning pilot light on a separator located approximately 5 feet away did not have the pilot tube cap installed which provided an ignition source for the flammable condensate, as determined on October 29, 2013. Abatement Certification Required for this Item.
