29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) Fast Track, Pod, Nurses Station: On or about October 30, 2013 and at times prior thereto, two (2) "Tripp Lite" relocatable power taps (RPT) were connected in series (daisy-chained) to provide power for the computer work stations. This condition of use was contrary to the associated UL listing and labeling when using the equipment and exposed employees to minor smoke-inhalation hazards in the event of RPT failure. (b) Fast Track, Pod, Practitioners Station: On or about October 30, 2013 and at times prior thereto, one (1) "Tripp Lite" relocatable power tap (RPT) was connected in series (daisy-chained) with a green model number CL-127 RPT to provide power for the computer work stations. This condition of use was contrary to the associated UL listing and labeling when using the equipment and exposed employees to minor smoke-inhalation hazards in the event of RPT failure.
