29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:    (a) Front side of building # 400 at 945 Crestmark Blvd, Lithia Springs, GA: Employees were exposed to fall hazards of approximately 28-feet while using a JCB 510-56 Telescopic Material Handler with a job-made scaffold platform which was not approved by the manufacturer to be used on this piece of equipment.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  (a) Inside Job made bin attached to JCB 510-56 Telescopic Material Handler at building #400, Lithia Springs, GA: Employees using an elevated scaffold work platform to access the roof located at heights of approximately 28-feet were not protected from falls to the paved surface below. Employees using the scaffold platform were not provided with guardrails or an anchor point to attach their personal fall protection systems.

29 CFR 1926.1053(b)(1):  A portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a) North side wall of building # 400 at 945 Crestmark Blvd, Lithia Springs, GA- The employer was using an extension ladder to access the roof of a building that was not extended three (3) feet above the landing exposing the employees to fall hazards.
