29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) Apartment Building #7, West side: On or about May 31, 2016, and at times prior, the employer exposed employees to struck-by hazards from the roof above, in that protective helmets were not worn while plywood and other materials were being installed directly above the roofing debris clean-up area on the ground level below.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Apartment Building #6, Lower roof: On or about May 31, 2016, and at times prior, the employer exposed employees to fall hazards of 10-feet, 10-inches from the lower ground level, in that the personal fall arrest system being worn by the employee was not connected to a life-line or anchor point while engaged in roofing activities.   (b) Apartment Building #7, Upper roof: On or about May 31, 2016, and at times prior, the employer exposed employees to fall hazards of 21-feet to the lower level on a 12/12 pitched roof, in that adequate fall protection was not being worn by employees engaged in roofing activities on a steep roof.  Home Life Roofing, LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 995980, citation number 2, item number 2 and was affirmed as a final order on 11/13/2014, with respect to a workplace located at 740 Sidney Marcus Blvd, Atlanta, GA.  Home Life Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 945480, citation number 1, item number 3 and was affirmed as a final order on 12/30/2013, with respect to a workplace located at 945 Crestmark Blvd., Bldg. #400, Lithia Springs, GA 30122.

29 CFR 1926.502(d): The employer used body belts as part of a personal fall arrest system: Note:  The use of a body belt in a positioning device system is acceptable and is regulated under paragraph (e) of this section:  (a) Apartment Building #7, Upper Roof: On or about May 31, 2016, and at times prior, employees installing plywood sheathing and installing roofing materials were exposed to asphyxiation and fall hazards, in that: body belts were being used as part of a personal arrest system.  Home Live Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.502(d)(17), which was contained in OSHA inspection number 995980, citation number 1, item number 1 and was affirmed as a final order on 11/13/2014, with respect to a workplace located at 740 Sidney Marcus Blvd. Atlanta, GA.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  (a) Apartment Building #6, Lower roof: On or about May 31, 2016, and at times prior, the employer exposed employees to fall hazards of 10-feet, 10-inches to the lower ground level, in that the ladder side rails were not extended at least 3-feet past the roof landing and the ladder was not secured to the structure.   (b) Apartment Building #6, Upper roof: On or about May 31, 2016, and at times prior, the employer exposed employees to fall hazards of 21-feet to the lower ground level, in that the ladder side rails were not extended at least 3-feet past the roof landing and the ladder was not secured to the structure.  Home Live Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 945480, citation number 1, item number 4 and was affirmed as a final order on 01/18/2013, with respect to a workplace located at 945 Crestmark Blvd., Lithis Springs, GA.
