WORKSHOP:  The hazardous energy control lockout/tagout procedures were generic and not written step-by-step in machine specifics for the employees during servicing and maintenance of equipment, such as (but not limited to) the CNC lathes and machining centers. 29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.  WORKSHOP:  The chuck of the LeBlond lathe was unguarded.     ABATED DURING THE INSPECTION.

29 CFR 1910.215(a)(2): Guard design. The safety guard shall cover the spindle end, nut, and flange projections. The safety guard shall be mounted so as to maintain proper alignment with the wheel, and the strength of the fastenings shall exceed the strength of the guard. 29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.  WORKSHOP:  The employer did not provide employees, who were required to work with chemicals, such as (but not limited to) oils and water soluable coolant with the training and information on the hazard communication program.
