29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   Employees operating blow mold machine number 7 to produce plastic cooler lids were exposed to caught-in/crush hazards in that; guard on the machine did not prevent employees from reaching the point where the die and ram stop come together, creating a pinch point.  The pinch point measured 19" from the front of the machine, and the opening measured 42.5" wide by 12.5" tall.  The bottom of the opening measured 36" from the floor.
