29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:     At the jobsite at 643 Clifden Drive, Saint Charles, Missouri, on or about October 28, 2013, an employee(s) using a ladder jack scaffold were not provided with or wearing a personal all protection system.  The fall distance to the concrete walkway below was approximately 15 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a/ At the jobsite at 643 Clifden Drive, Saint Charles, Missouri, on or about October 28, 2013, the side rails belonging to portable extension used to access the roof from the front of the house was extended only one rung (approximately 12 inches) above the eave.
