29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest system.  On or about October 29 10, 2013,  employees who were performing decking activities were not provided fall protection.  The employees were exposed to a fall hazard at a height of over 6 feet to the ground below.

29 CFR 1926.1052(c)(1)(i):  Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail.  On or about October 29, 2013,  employees who were performing decking activities were using a stairway without a handrail. The employees were exposed to falls of 10 feet to the ground below.

On or about October 29, 2013, the employer did not ensure employees engaged in framing activities were trained to recognize and avoid hazards related to ladders and the procedures to be followed in order to minimize or avoid those hazards.
