The wall openings on the second floor were not guarded to prevent employees from falling. Employee(s) exposed to a thirteen feet fall. Employees were descending down through the rafters to the top step of the ladder that was five feet tall. Employee(s) were observed stepping on the top rung of the step ladder. The Werner step ladder was used in a manner that is prohibited by the manufacturer.
