29 CFR 1910.134(h)(1): Respirators were not cleaned and disinfected using the procedures in Appendix B-2 of 29 CFR 1910.134 or equivalent procedures recommended by the respirator manufacturer:  a. Establishment - As of 10/31/2013, the employer had not ensured that respirators were cleaned and disinfected at appropriate intervals using approved procedures as designated by this paragraph. The inside of a respirator used by an employee was heavily contaminated with lead containing dust.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or  were not packed or stored to prevent deformation of the facepiece and exhalation valve:  a. Establishment - As of 10/31/2013, the employer had not ensured that respirators were stored appropriately. A respirator used by an employee when cutting lead sheathed cable with an abrasive disc saw, was stored in a tool box where the storage bag could become contaminated, and where the facepiece of the respirator could be subject to deformation or damage.

29 CFR 1910.134(h)(4): Respirators that failed an inspection or were otherwise found to be defective were not removed from service and discarded or repaired:  a. Establishment - As of 10/31/2013, a respirator was used by an employee when cutting lead sheathed cable. It had discolored and curling inhalation valves and the lining inside the facepiece was peeling. It was not repaired or removed from service.

29 CFR 1910.1025(d)(6)(ii): Where the initial determination or subsequent monitoring revealed employee exposure to lead to be at or above the action level but below the permissible exposure limit; monitoring was not repeated at least every six months:    a. Warehouse - As of 10/31/2013, monitoring for lead was not repeated as required. Monitoring done on 05/23/2008 for an employee cutting the 1 in. lead sheathed cable showed an exposure at the action level. This job continues currently and no further monitoring has been done.
