29 CFR 1910.263(i)(7)(i): Wherever a conveyor passes over a main aisleway, regularly occupied work area, or passageway, the underside of the conveyor shall be completely enclosed to prevent broken chains or other material from falling in the passageway. a. Line Packing Area- On or about November 6, 2013, conveyor was not sufficently enclosed. The underside of conveyor belt was sagging significantly below the guard. Thereby exposing employees to caught-in hazard(s).

Safety of equipment shall be determined using the following considerations: such as electrical insulation. a. Production Line- On or about November 6, 2013, Employees were required to operate a pendant where the outer jacket and insulation was badly worn and also revealed exposed conductors. Thereby exposing employees to shock and electrocution hazard(s).
